HANYS awarded Mother Cabrini Health Foundation grant for Advancing Healthcare Excellence and Inclusion initiative

Nearly $1 million in funding will enable HANYS to deepen and target its work in health disparities

RENSSELAER, NY — With generous support from a grant of the Mother Cabrini Health Foundation, the Healthcare Association of New York State is launching a new Advancing Healthcare Excellence and Inclusion initiative.

As part of this project, HANYS will leverage data from members and partners to develop a baseline understanding of communities/regions most in need of interventions related to social determinants of health. HANYS will then work with hospitals and community partners in three targeted communities/regions in the state to:

  • improve the collection of hospital patient demographic data using the “We Ask Because We Care” framework;
  • form and/or strengthen local community coalitions; and
  • co-design a community-specific analysis with local community-based organizations, providers and other local leaders to identify opportunities to impact health equity.

To support these goals, HANYS will provide data analytics; a virtual education series; communications and marketing materials; and tailored technical assistance from seasoned experts in health equity, change management, hospital operations and coalition building.

HANYS, our board of trustees and our members are focused on addressing healthcare disparities. The disproportionate impact of COVID-19 on vulnerable communities has made this work even more urgent. The Foundation’s support will allow us to deepen and target our high-value education, data analytics and technical assistance in communities across the state to build bridges as a trusted partner,” said Bea Grause, RN, JD, president, HANYS.

“The Mother Cabrini Health Foundation is committed to improving the health and well-being of vulnerable New Yorkers by bolstering the health outcomes of diverse communities, eliminating barriers to care and bridging gaps in health services. We are pleased to support HANYS’ continued work to improve health equity as part of our strategic grant making portfolio,” said Daniel Frascella, chief programs and grants officer, Mother Cabrini Health Foundation.

HANYS’ clinical and policy professionals manage a number of collaboratives and support networks focused on improving quality, safety and outcomes for healthcare organizations across the continuum of care. They’re excited to launch the Advancing Healthcare Excellence and Inclusion initiative this month.

HANYS thanks the Mother Cabrini Health Foundation for its continued partnership in improving healthcare for all New Yorkers.
