This December: Join HCA-HANYS-IHA for two webinars highlighting innovative hospital and home care collaborations
Mark your calendar for Dec. 3 and Dec. 17, from 4 - 5 p.m. to join HCA-HANYS-IHA. They’ll explore successful strategies for providing high-quality patient care no matter the setting as part of the Statewide Hospital-Home Care Collaborative for COVID-19 and Beyond, supported by a grant from the Mother Cabrini Health Foundation.
Sessions are designed to promote pre- and post-hospital collaboration among hospitals and home care providers to address patient needs not only in the COVID-19 pandemic, but on a lasting basis.
The Dec. 3 webinar will feature Mount Sinai South Nassau and South Nassau Home Care and their focus on care transitions and recovery from end-stage chronic obstructive pulmonary disorder, diabetes, pre-and post-operative care in orthopedics or open heart surgeries. Add the upcoming Dec. 3 and Dec. 17 sessions to your calendar!

HANYS recognizes our members across the state for their dedication to improving antibiotic prescribing and reducing drug resistant infections during National Antibiotic Awareness Week, Nov. 18-24.
Visit the HANYS Antibiotic Stewardship Collaborative webpage for toolkits, guidelines and educational materials. Member Spotlight: St. Mary’s Healthcare in Amsterdam strengthens food security
Food insecurity is one of the most significant social determinants affecting community health. Lack of access to healthy foods adds to the root causes of poor health outcomes. The economic instability resulting from the ongoing COVID-19 pandemic has only exacerbated this issue in many communities. In 2018, St. Mary’s Healthcare began its Community Food Drop and Medical Mission to improve food security and access to healthy food and beverages for vulnerable people across the service area.
