Pilot Initiative for Community Health Improvement

The Public Health Foundation (PHF), in collaboration with the Centers for Disease Control and Prevention (CDC), Association for Community Health Improvement/Health Research and Educational Trust, and Catholic Health Association, is engaged in a pilot initiative aimed at helping hospitals and health systems use The Guide to Community Preventive Services (The Community Guide). This free resource, which provides evidence-based recommendations and findings from the Community Preventive Services Task Force, details the ways hospitals and health systems can take action and implement community health improvement activities.

The pilot initiative focuses on developing and enhancing informational training materials related to using The Community Guide to address population health priorities. The first training event, a webinar, introduced the population health driver diagram framework, which is currently being used by hospitals, health systems, and their community partners during field testing to align and implement actions to address identified population health priorities.
