HANYS Providing Free NYS PCMH Technical Assistance to Primary Care Practices
HANYS is pleased to provide free technical assistance for primary care practices seeking New York State Patient-Centered Medical Home (NYS PCMH) recognition.
New York State, in collaboration with the National Committee for Quality Assurance (NCQA), has developed the NYS PCMH recognition program. This state-specific recognition aligns the priorities of New York State with NCQA’s PCMH recognition, including a focus on behavioral health integration, health information technology, care coordination and care management, and value-based payments.
The State Innovation Model (SIM) grant awarded to New York allows HANYS to partner directly with primary care practices across the state, and guide them through NYS PCMH transformation. In addition, New York State will sponsor practices’ first year NYS PCMH recognition fee, or the first NYS PCMH annual reporting fee. Technical assistance is available for physician-led and nurse practitioner-led practices.
