Enrollment open for Age-Friendly Health Systems New York State Action Community
Enrollment is now open for the HANYS Age-Friendly Health Systems New York State Action Community. This program supports New York hospitals, health systems and providers across the continuum of care as they work to implement the Institute for Healthcare Improvement’s 4M framework and transform the care delivered to older adults.
Participating teams will work together for about seven months. All education, coaching and networking events will be held virtually. Teams will work at their own pace to implement the 4Ms and can earn recognition from IHI for reaching specific milestones.
Thanks to generous support from the Fan Fox & Leslie R. Samuels Foundation, the New York State Health Foundation and the Health Foundation for Western and Central New York, there is no fee to participate in the New York State Action Community.
An upstate cohort will begin in November followed by a downstate cohort in February. Facilities are invited to join either cohort based on internal readiness, regardless of geographic location.
