CMS releases funding opportunity for rural transformation grants
This week, CMS released the Notice of Funding Opportunity for the previously announced Community Health Access and Rural Transformation Model.
The agency is providing funding for rural communities to build systems of care through a Community Transformation Track and is encouraging providers to participate in value-based payment models where they are paid for quality and outcomes, instead of volume, through an Accountable Care Organization Transformation Track.
Up to 15 awards of a maximum of $5 million each will be made nationwide for the program, which is expected to cover a seven-year performance period. Eligible applicants include local, county and state governments, not-for-profit entities, faith-based organizations, for-profit entities other than small businesses and federally recognized tribal organizations.
A letter of intent is due Jan. 18, with the full application due Feb. 16. Contact: Karen Roach
