Care Connections highlight: Rochester Regional Health - St. Lawrence Region self-management workshop supports patients with chronic disease

In 2025, Rochester Regional Health joined Care Connections, a HANYS learning collaborative, to further develop its community partnerships. Care Connections provided data reports on chronic disease prevalence in the hospital’s service area, identified high-priority chronic diseases and helped coordinate work with The Heart Network.
