New reports recommend continued collaboration to improve patient care, outcomes

As part of the Statewide Hospital-Home Care Collaborative for COVID-19 and Beyond, funded by the Mother Cabrini Health Foundation, the Home Care Association of New York State, HANYS and Iroquois Healthcare Association released four new reports this week. The reports detail best practices to meet the growing demand for mental health services, public vaccination efforts, partnering with the agency services network and establishing new models of hospital-home care innovation.
