NYS Payment Innovation Symposium

Government is placing a strong focus on value-driven payment models. At the federal level, the Center for Medicare and Medicaid Innovation is sponsoring numerous pilot and demonstration programs around shared savings, accountable care organizations (ACOs), and bundled payment–most recently the mandatory Comprehensive Care for Joint Replacements payment program for the New York and Buffalo areas.

At the state level, the Medicaid Delivery System Reform Incentive Payment (DSRIP) program requires that 90% of Medicaid managed care be transitioned to Value Based Payment (VBP) by 2019. The DSRIP VBP mandate will require managed care organizations and Performing Provider Systems to negotiate for comprehensive payment amounts and meet quality metrics to retain any shared savings.

Many healthcare providers, particularly hospitals and physicians, see payment innovation as a necessary component for population healthcare–without a sustainable payment stream, such as an opportunity to share in savings or payments for care management, providers cannot finance the changes and supports necessary to truly redesign the healthcare delivery system. However, along with opportunities to redesign care delivery, new payment methods bring unique challenges that healthcare providers need to understand and address.
