HANYS is pleased to continue our Complex case discharge delay learning series with a program focused on surrogate decision-making.
New York’s Surrogate Decision-Making Committee program is an alternative to the court system and is authorized to provide consent for non-emergency major medical treatment and end-of-life care decisions for qualifying individuals. Eligible individuals include those who currently reside or previously resided in an Office for People with Developmental Disabilities, Office of Mental Health or Office of Addiction Services and Supports program, and/or receive OPWDD services for major medical treatment decisions. For end-of-life care decisions, eligibility is based on a diagnosis of intellectual/developmental disability

During this webinar, attendees will learn how the SDMC can support care transitions, including eligibility and the application process.
