Setting the standards: Caring for people with dementia in hospital settings

More than 400,000 New Yorkers live with Alzheimer’s disease and related dementias. To help providers across the continuum of care better meet the needs of these patients and their families, HANYS and the Alzheimer’s Association developed a three-part webinar series, Modernizing dementia: The new dementia care landscape, featuring case studies from New York hospitals that address best practices and new developments in treatment.

Acute care teams are treating more older adults with multiple and complex conditions. Addressing dementia can positively impact multiple quality measures in the hospital setting. This session will demonstrate the impact of a dementia diagnosis on quality care measures related to age-friendly health systems (e.g., mentation, mobility, medication, what matters). The session will also provide proven interventions to improve outcomes for people with dementia in the hospital setting.
