NYS PCMH Practice Transformation––Strategies for Success

The Healthcare Association of New York State (HANYS) invites you to NYS PCMH Practice Transformation––Strategies for Success. The complimentary program begins with cocktails and registration at 5:30 p.m., followed by a sit-down dinner and presentations from industry experts.

The New York State Department of Health (DOH) and National Committee for Quality Assurance (NCQA) launched the NYS PCMH program in April. Through the State Innovation Model (SIM) grant, primary care practices may obtain grant-funded technical assistance under the guidance of Practice Transformation Assistance (PT TA) vendors. HANYS is a PT TA vendor for Long Island. Practices are eligible for up to 15 months of free support, depending on current recognition status.

Why NYS PCMH Recognition?
In today’s rapidly changing healthcare reform environment, multiple programs and initiatives are available to primary care practices. We will discuss the role of practice transformation in positioning for success in Value-based Payment (VBP) as well as other regional, statewide, and federal programs.

The Physician’s Perspective
Hear about challenges and triumphs of practice transformation from a panel of Long Island primary care physicians who are either working toward or have achieved NYS PCMH Recognition.

The Technical Assistance Program
Free? Who doesn’t like free? We will introduce attendees to NYS PCMH and the free technical assistance available through the SIM grant.

Continuing Education Credits
There are no continuing education credits associated with this Webconference.
