Complex Case Discharge Delay Learning Series: Leveraging the Data

HANYS is pleased to continue our Complex Case Discharge Delay Learning Series with a session focused on data. During this program, Amanda Laprime, PhD, director of the Applied Behavior Analysis Care Team at University of Rochester Medical Center, will share ways her health system uses data to understand and tackle complex case discharge delays.

HANYS Complex Case Discharge Delay Learning Series is focused on highlighting possible solutions to help our hospitals address the myriad of issues associated with complex case discharge delays. The series features many of the themes identified by members, including sessions focused on guardianship, best practices for EDs caring for individuals with intellectual disabilities, immigration and legal status, and other topics.

Recordings of previous sessions:

  • URMC project ECHO geriatric mental health - University of Rochester Medical Center discussed integration of behavioral health services into skilled nursing facilities, and establishment of tele ECHO clinics that connect URMC specialists with SNF sites across New York.
  • Caring for individuals with intellectual an developmental disabilities - Leaders from Metro Community Health Centers in Bronx, NY and Center for Disability Services in Albany, NY reviewed levels of support available to individuals with I/DD, discharge obstacles and recommendations for transitions of care, including challenges unique to the COVID-19 pandemic.
  • Navigating the guardianship process - expert in guardianship, John Holt, JD of Project Guardianship, provided an overview of guardianship in New York State and shared insight into navigating the process.
