Partnering to improve dementia care: Alzheimer's Association and New York providers

This event is open to HANYS members only.

This webinar will provide an overview of the many challenges experienced by people living with dementia and how they can successfully connect with health systems and the community at large. The Alzheimer's Association and hospital clinicians will share how they work to improve quality care outcomes, reduce costs and improve patient and staff experiences.

By the end of this webinar, you will be able to:

  • list at least three ways people living with dementia experience challenges in acute and primary care settings; and
  • understand how two health systems are working with the Alzheimer's Association to improve quality care for people with dementia.
