Hypertension (high blood pressure) can lead to heart disease, stroke, and many other medical complications. Yet many people have hypertension that goes untreated. In an effort to address the rising incidence of hypertension, Albany Medical Center is working to improve the coordination of care between Albany Medical Center physicians and primary care physicians throughout the region.
Through this project, primary care physicians are receiving prompt notification if their patient presents with high blood pressure to any Albany Medical Center physician or caregiver--from any specialty, in any setting. Working together, Albany Medical Center physicians and primary care physicians develop a coordinated treatment plan to address all of the patient's conditions and needs. Concurrently, Albany Medical Center has developed a "Know Your Numbers" campaign to promote awareness of the dangers of hypertension for all of its 8,000 employees.
