With primary care being one of the most important building blocks for healthy communities, New York's hospitals and their physicians have enthusiastically embraced the PCMH model. Glens Falls Hospital is an excellent example of a hospital bringing PCMH to its community. Over the past two years, the hospital's 14 primary care physician practices made the transformation to Level III PCMH.
