Long Island Community Hospital Integrates Medical and Behavioral Healthcare

Medical and behavioral health services have traditionally been delivered independent of each other, often leading to care fragmentation. To prevent this, Long Island Community Hospital (formerly Brookhaven Memorial Hospital Medical Center) and the Brookhaven Family Medicine Center recently developed a patient-centered medical home (PCMH) that integrates medical and behavioral healthcare. This PCMH provides primary care, identifies patients who are at risk for mental illness, connects patients to medical providers in the community, and provides the opportunity to train future physicians on how to provide integrated primary care.

In addition to providing more patient-centered care, Long Island Community Hospital anticipates that this PCMH will reduce the number of visits to the Emergency Department and decrease readmissions.
