AHA Issues Social Determinants of Health Guide
The American Hospital Association (AHA) released a one-page strategic implementation guide that offers three general paths providers can take to address the underlying social conditions affecting their patients. It also suggests federal funding options through Centers for Medicare and Medicaid Services (CMS) that would allow hospitals to create bridge organizations in their communities to assist in screenings and clinical delivery. In addition, the guide contains resources providers can reference, including AHA’s ongoing Social Determinants of Health Series.
This guide is a valuable resource for hospitals and healthcare systems to share with their governing boards. The guide was developed as part of AHA’s agenda to help hospitals and healthcare system boards address the underlying social conditions affecting vulnerable communities’ access to healthcare and underachieved health goals.
