Partnering to Achieve Sustainable Practice Transformation
The annual statewide Learning Symposium convened the Delivery System Reform Incentive Payment (DSRIP) program Performing Provider Systems (PPSs) February 6-8 at the Hilton Garden Inn, Staten Island. The program was attended by the 25 PPSs, along with partners, stakeholders, state leaders, and national experts in healthcare and system transformation.
HANYS Solutions Practice Advancement Strategies and NewYork-Presbyterian-Queens PPS teamed up to submit a proposal and were selected to participate in this year’s poster reception. Fifty-three posters were on display throughout the Symposium, representing the improvement efforts taking place throughout the state. Partnering to Achieve Sustainable Practice Transformation outlined how NewYork-Presbyterian Queens PPS forged a partnership with HANYS Solutions Practice Advancement Strategies to bring practice transformation to not only the NewYork-Presbyterian Queens system-owned practices but also to community partners. It included NewYork-Presbyterian Queens PPS risk-mitigation strategy and learning collaborative approach to achieve National Committee for Quality Assurance patient-center medical home (NCQA PCMH) Level 3 standards.
