Upgrade to NYS PCMH

Registration is open to HANYS Members and Non-Members.
Join HANYS Solutions Practice Advancement Strategies on Thursday, July 19, at noon, for Upgrade to NYS PCMH. The 60-minute, complimentary webinar will discuss the steps necessary to upgrade to NYS PCMH for existing recognition practices, including any level of PCMH 2011 or 2014, and also for PCMH 2017.
Learning objectives:

discuss the requirements and considerations for practices with current NCQA PCMH recognition to transition to NYS PCMH;
-
identify the Annual Reporting and 12 additional core criteria requirements; and
-
understand the benefits of transitioning to NYS PCMH with support of Practice Transformation Technical Assistance vendor through the State Innovation Model (SIM) grant.
