Implementing the Planetree Model in New York: Lessons Learned from a Yearlong Collaborative

Speaker Jim Kinsey, director, member experience, Planetree International, will provide an overview of Planetree’s evidence-based framework for patient- and family-engaged care, share key insights from the recent HANYS-Planetree Patient Experience Collaborative and identify additional educational opportunities for HANYS’ members under the HANYS-Planetree partnership.

In 2017, HANYS formed a partnership with Planetree, an international organization with more than 40 years of experience in helping healthcare organizations deliver patient-centered care. Twenty-four hospitals from across the state participated in HANYS’ yearlong collaborative to implement the Planetree model. This evidence-based approach to person-centered care yields internal, external, financial and cultural benefits to healthcare organizations. It engages patients, aligns with mandates for better population health management and improves organizational performance in quality, safety and experience.

HANYS’ C2X™ program provides a dynamic portfolio of programs designed to support hospital-specific improvement efforts.
