Improving High-Risk Patient Outcomes Through an Acute and Post-Acute Provider Collaborative

Leaders from Metropolitan Jewish Health System (MJHS) Home Care and SUNY Downstate Medical Center will review their collaborative initiative that coordinates acute and post-acute care and has improved outcomes for high-risk, chronically ill patients. This innovative delivery system model is transferable to a wide range of chronic care populations and addresses many of the expectations for health delivery reform in the Affordable Care Act of 2010.

The initial MJHS/SUNY Downstate collaboration served patients recovering from renal transplant surgery. Representatives from both facilities share how their partnership began, the added value contributed by each setting and how regulatory and reimbursement barriers were overcome.
