Adirondack Health Collaborative Keeps Seniors in Their Communities

More and more seniors living in Adirondack Park communities want to get the health services they need while continuing to live in their own homes. To address the North Country's need for innovative nursing home alternatives, Adirondack Health's two nursing homes in Lake Placid and Tupper Lake joined together with Adirondack Tri-County in North Creek and Heritage Commons in Ticonderoga to form the Blue Line Group.

These four facilities are working together to develop innovative collaborations, efficiencies, and shared services so their seniors can receive the coordinated care they need while still living safely in the community. One such efficiency the Blue Line Group is working towards is a single electronic health record for each patient.
