44 HANYS-supported New York facilities achieve IHI Age-Friendly Health System designation; recruitment for 2025 cohort begins Nov. 13

Since 2020, 160 healthcare organizations earned Age-Friendly designation with support of the HANYS-led Action Community

HANYS is celebrating 44 New York healthcare providers’ recognition as Level 1 or Level 2 Age-Friendly Health Systems by the Institute for Healthcare Improvement. These providers participated in the HANYS-led New York State Age-Friendly Action Community 2024 cohort.

HANYS continues to grow New York’s Age-Friendly movement

HANYS launched the New York State Age-Friendly Action Community in 2020 with support from the New York State Department of Health, The John A. Hartford Foundation and IHI. The Action Community helps participating health systems improve care for older adults using an “all teach, all learn” model.

Since its inception, 160 healthcare organizations have achieved Age-Friendly recognition as part of HANYS’ Action Community.

“As we said in The Case for Change, our healthcare system must be redesigned to address the needs of our changing, aging population,” said Bea Grause, RN, JD, president, HANYS. “Our Age-Friendly program and the tremendous work of our dedicated participants is helping to ensure older adults statewide are receiving age-appropriate, compassionate care wherever they interact with the healthcare system, whether that happens in a primary, acute or long-term care setting.”

Across the state, more than 300 care settings have achieved Age-Friendly recognition to date, making New York second in the nation for the number of healthcare settings with IHI's designation.

Age-Friendly care recognized by CMS as best practice

Age-Friendly Health Systems use a set of four evidence-based elements to organize the care of older adults, known as the “4Ms”: What Matters, Medication, Mentation and Mobility. Age-Friendly facilities, including our Action Community participants, report improvement in patient experience scores and quality outcomes such as readmissions, emergency department utilization and length of stay.

In 2024, CMS adopted the Age Friendly Hospital Measure to evaluate and publicly report on hospitals’ ability to provide Age-Friendly care. The five domains of the measure align with the Age-Friendly Health Systems framework.

Healthcare providers encouraged to join 2025 cohort

Providers across the continuum of care are invited to join our 2025 New York State Age-Friendly Action Community cohort. Over 10 months beginning in January, participating sites will receive:

  • virtual education from HANYS staff and expert faculty;
  • tested strategies and best practices for implementing of Age-Friendly 4Ms care;
  • peer-sharing and opportunities to meet other Age-Friendly participants, learn from one other and accelerate the adoption of best practices;
  • opportunities to apply for stipends to support implementation of 4Ms care; and
  • technical assistance with their applications for official IHI Age-Friendly Health Systems recognition.

There is no fee to participate thanks to generous support from the Fan Fox & Leslie R. Samuels Foundation, New York State Health Foundation, Health Foundation for Western and Central New York, and The New York Community Trust.
