Mohawk Valley Health System Improves Patient Safety

After identifying opportunities for improvement through the Agency for Healthcare Research and Quality Culture of Safety Survey, Mohawk Valley Health System adopted the National Quality Forum’s Safe Practices as the framework for developing a formal, regular process for patient safety rounds.

The “Safety First Friday Rounds” program aims to increase identification of actual and potential patient safety problems, increase leadership interaction with staff and patients, and advance the value of safety and excellence. The program provides opportunities to solicit real-time input from patients and their families about hospital safety and promotes immediate action for change. All leaders participate in a two-hour timeframe each Friday to deliver educational safety-focused presentations, perform hospital and outpatient office audits with staff, and recognize employees for their participation and suggestions.

Since the program began, appropriate identification of isolation patient equipment rose from 68.5% to 85.3% and dating of multi-dose vials improved from 61.5% to 95.8%.
