Age-Friendly Health Systems Action Community recruitment event

Enrollment is open for the 2021-2022 New York State Age-Friendly Health Systems Action Community. The Action Community will support hospitals and health systems as they work to implement evidence-based interventions to transform care for older adults. The IHI Age-Friendly care model focuses on four key areas of improvement, known as the 4Ms: What Matters, Medication, Mentation, and Mobility.

Join HANYS for a recruitment webinar with IHI Age-Friendly leader Dr. Donald Berwick, who will provide an overview of the Action Community and the benefits of participation.

Participating teams will work together for approximately nine months to test and adopt the 4Ms framework across their care setting. Facilities participating in the New York State Action Community will benefit from:

  • support from national and state faculty, including quality improvement coaches and experts in Age-Friendly care;
  • technical assistance, including regular team meetings, coaching calls and support to implement and test new interventions;
  • access to a state and national network of Age-Friendly Health Systems and opportunities to share strategies to expedite the implementation of the Action Community goals; and
  • quality improvement knowledge that can be applied beyond the scope of this initiative.

This work is especially important now, given the experience of older adults and their families during the COVID-19 pandemic. The New York State Action Community will draw on lessons learned during the pandemic and will use this context to guide participating health systems as they improve care for older adults.

Thanks to the generous support of the Fan Fox & Leslie R. Samuels Foundation, the New York State Health Foundation, the New York Community Trust and the Health Foundation for Western & Central New York, there is no fee to participate in the New York State Action Community. Health systems may apply through HANYS for a $10,000 stipend to support this work. Teams may work at their own pace to implement the Age-Friendly model of care.
